Viewing Assessments and the Service Plan

FamilyFamilyPhysicianUpdated Jul 26, 2026

Overview

As part of your loved one's care, the team completes assessments — structured reviews of health, needs, and goals. These assessments often shape your loved one's care and service plan. When the care team wants you to see or take part in an assessment, they share it with you, and it appears in your Forms inbox in the Family Portal.

This article explains where to find assessments that have been shared with you, how to read them, and how assessments relate to your loved one's service plan.

Before You Begin

  • You are signed in to the Family Portal.
  • The care team has shared an assessment with you. If nothing has been shared, your Forms inbox will not show one.

Steps

Step 1: Find Shared Assessments in Your Forms Inbox

Assessments are shared through the same place as forms. Select Forms at the bottom of the screen to open your inbox, or open the Forms section on your loved one's page to see only their items. Each item shows its name, the date, and a status of Pending or Reviewed.

Step 2: Open an Assessment

Select the assessment to open it. The top of the screen shows the assessment name, your loved one's name, and its status. If the assessment has already been completed by the care team and shared for your review, it opens as a read-only view you can scroll through.

Step 3: Read the Assessment Details

Scroll through the assessment to read each section and the recorded answers. For a completed assessment, this is where you can see the details the care team documented — the observations, needs, and goals that inform your loved one's care.

Step 4: Complete an Assessment If Asked

Some assessments are shared so you can add your own input — for example, your observations or a signature. When that is the case, you move through the assessment page by page and submit it, just like any other form. For the full steps, see Reviewing and Completing Forms.

Step 5: Understand the Service Plan

Your loved one's service plan (also called a care plan) is built from these assessments — the completed assessment reflects what the care team has planned. The Family Portal does not have a separate service-plan page, so the assessment you review is where you see this information. If you want to talk through the plan itself or ask about goals and services, contact the care team directly.

Tips

  • Look under the Pending tab for assessments that still need your review or input, and the Reviewed tab for ones you have already submitted.
  • A completed assessment is the plan in detail. Reading it is the best way to understand the care and services the team has planned.
  • Bring questions to the team. The portal shows you the assessment, but the care team can explain the reasoning and next steps.

Troubleshooting

Problem Solution
I do not see any assessments None have been shared with you yet. Assessments only appear in your Forms inbox when the care team shares them. Contact the center if you expected one.
I cannot find a separate "care plan" or "service plan" page There is no separate plan page in the portal. The plan is reflected in the completed assessment shared with you. Contact the care team to discuss the plan itself.
The assessment is read-only It was shared for your review only. If the team wants your input, they will share it with editing access.
I have a question about something in the assessment Contact the care team. They can explain the findings and how they affect your loved one's care.
