Overview
The Family Portal gives you a view-only window into the health information the care team keeps for your loved one. There is no single "Health" page — instead, your loved one's page has separate sections for Diagnoses, Allergies, and Medications. This article shows you how to open each one and what you will see.
This information is maintained by the center's care staff. If anything looks incorrect or out of date, contact the center — you cannot edit it from the portal.
Before You Begin
- You are signed in to the Family Portal and have chosen your loved one from the Members list.
- The center has entered the health information you are looking for.
Steps
Step 1: Open Your Loved One's Page
From the Members list, select your loved one. Their page opens with their photo and summary at the top, and a list of sections underneath.

Step 2: View Diagnoses
Select Diagnoses to see the conditions on file. Each condition appears as a card showing:
- Name — the condition or diagnosis
- Physician — the physician associated with the condition, if recorded
- Notes — any notes the care team has added

Step 3: View Allergies
Go back and select Allergies. Each allergy appears as a card showing:
- Name — what your loved one is allergic to
- Severity — how serious the allergy is
- Type — the kind of allergy (for example, medication or food)
- Description — more detail about the allergy
- Reaction — what happens during a reaction
- Intervention — the recommended response or treatment
Allergies marked as severe are highlighted so they stand out.

Step 4: View Medications
Go back and select Medications. Each medication appears as a card showing:
- Name — the medication
- Days — the days it is scheduled
- Times — the times it is given
- Center Served — whether the medication is given at the center
Screenshot coming soon — The Medications section shows days, times, and where each is given
Tips
- Check before appointments. Reviewing the medication list and diagnoses before an outside doctor's visit helps you answer questions accurately.
- Pay attention to severe allergies. These are highlighted for a reason — knowing them helps you share critical information in an emergency.
- Keep your own copy. If it helps, note down the medication list so you have it handy when you are away from the portal.
Troubleshooting
| Problem | Solution |
|---|---|
| I do not see a Diagnoses, Allergies, or Medications section | Make sure you have opened your loved one's page from the Members list. If a section is still missing, contact the center to confirm your account is linked correctly. |
| A section is empty | The care team may not have entered this information yet. Contact the center if you expected to see something. |
| The information looks out of date | Health records are managed by the center's care staff. Contact the center to report anything that looks wrong, and share any documentation from recent physician visits. |
| I want to add or change something | The portal is view-only. Ask the center's care team to make any updates for you. |
