Reviewing and Completing Forms

FamilyFamilyPhysicianUpdated Jul 26, 2026

Overview

From time to time the care team will share a form or assessment with you through the Family Portal — for example, an intake form, a questionnaire, or a document that needs your signature. These arrive in your Forms inbox.

Some forms are shared for you to read only. Others are shared for you to fill in and send back. This article shows you how to find your forms, complete the ones that need your input, sign where required, and submit them.

Before You Begin

  • You are signed in to the Family Portal.
  • The care team has shared a form or assessment with you. They may let you know when something is waiting.

Steps

Step 1: Open Your Forms Inbox

Select Forms at the bottom of the screen to open your inbox. It lists every form shared with you across all your loved ones. You can also open the Forms section on a specific loved one's page to see only their forms.

Each item shows the form's name, the date, the loved one it belongs to, and a status label.

Step 2: Understand the Tabs and Statuses

Your inbox has three tabs: All, Pending, and Reviewed (it opens on Pending). The status label on each form tells you where it stands:

Status Meaning
Pending The form is waiting for you to review or complete.
Reviewed You have already submitted the form. It shows as Submitted when you open it.

Step 3: Open a Form

Select any form to open it. At the top you will see the form name and your loved one's name, along with the current status. If the form was shared for reading only, it opens as a complete, read-only view — you can scroll through it, but there is nothing to fill in.

Step 4: Complete the Form

If the form was shared for you to complete, answer each question as you move through it. Depending on the form, you may enter text, choose from options, pick a date, or fill in other details. Use Next to move forward and Back to return to an earlier page. Required questions must be answered before you can submit.

Step 5: Sign and Review

After the last page, you reach a review page that shows your answers together. If the form asks for your signature, add it here. Any required questions or signatures you still need to complete are called out so you can fix them.

Step 6: Submit for Review

When everything required is complete, select Submit for Review and confirm. The form's status changes to Submitted, and the care team can see that you have sent it back. The button stays unavailable until all required questions and signatures are filled in.

Tips

  • Start with the Pending tab. It shows exactly what still needs your attention. Anything you have finished moves to Reviewed.
  • You do not have to finish in one sitting. Your answers are saved as you go, so you can return later to complete a form.
  • If Submit for Review is unavailable, a required question or signature is still missing. The form points out what is left.
